I will second the pillows under the butt, that kind of puts the belly up and back a little. Helps with the configuration of things.

Second thought is maybe a modified from the side approach, one leg up, and you approach from the side. This also puts the belly over to the side and opens up the access way.

As for the "doggie". If her knees are bad, which lots of us big gals expierence. Try pillows under her belly. Firmer long pillows seem to work best. That will take the pressure off of her knee.

Okay now I am blushing.
